A Positive BHCG But Now Bleeding Two Weeks After Embryo Transfer: Am I Miscarrying?
Question:
Hello. I did an IVF transfer June 24th, was told it was a beautiful transfer of 3 beautiful embryos. This past Tuesday, July 6th, I got a positive result, however doc told me to be cautiously optimistic as it could be a chemical pregnancy because my level was 33. The next morning I woke up to cramps and started brown (old blood like) spotting, which is kind of weird for me to be honest. My cautious optimism diminishes to despair. Called doc, told me it was normal and had to wait. Retested 48 hrs later (yesterday) and it was 60, so it nearly doubled! Which was a big shocker...I was really expecting it to go down. But yeah, right?
This morning, July 10th, I woke up to heavier spotting and an actual clot like thing (which kind of looked like a massive amount of my Crinone gel). Mostly brown blood with a little red now, but mostly when I wipe after going to the bathroom (sorry TMI). They say it could be a number of things, irritation from the progesterone gel, passing of one of the embryos, or just normal pregnancy symptoms. I think they are miscarry symptoms. I go back for blood draw Monday and I am just beside myself. I am normally a pretty together person but this has turned me upside down as I feel like I'm on an emotional roller coaster. One day I think I am dealing with a m/c and then we get news that my hcg doubles. This bleeding after my embryo transfer worries me.
I guess what I'm asking is should I look at the doubling as good news or could it be my levels haven't caught up with what will happen? Could I be passing one or two embryos while one implants? Is a m/c imminent in your opinion? Any help would be most appreciated. Thank you in advance. D. from the U.S.

First, let me answer your last question. Based on your symptoms and bHCG levels, you CANNOT make a diagnosis of imminent miscarriage. It can go either way, but I tend to focus on the good side.
The bHCG levels doubled as they were supposed to. I generally follow them every 48 hrs for the first four levels, but your doctor has a different protocol. As long as they continue to rise and approximately double (they don't have to double exactly), then we wait and see how things go. There is only a limited amount of information that you can gather from this blood test. If they are going up, you are pregnant and things are looking good. If they plateau or drop then things are not good. One cannot necessarily predict the outcome based on the levels.
Second, I would totally disregard the spotting that you are having, and not worry unless it is bright red blood that flows like a period. Anything less than that can be "normal" at this point. Bleeding after embryo transfer is more common than you think. We see this very, very often. . . in almost 90% of our IVF patients using Crinone. In fact, there have been studies showing increased spotting with vaginal progesterones like Crinone. It is thought that the progesterone makes the cervix more prone to oozing from the surface. In any case, this type of spotting with Crinone is very common.
Not much can be known about the pregnancy at this very early point, except that you are pregnant! Now, despite the worry, you have to wait and see how things progress. Try to be optimistic and keep the stress level down. I would not assume that you are destined for a miscarriage or are miscarrying yet. It can still go either way. Whatever is destined to happen, will happen and we don't have the power to change that at this point. We can only hope and pray for the best. By the way, it is never "TMI" (too much information), rather, all the information you can provide is appreciated by a physician.

Very thoughtful and thorough reply. That's the first time I've come across the connection between Crinone and bleeding. I've had spotting and intermittent seeping since day 16 after HPT. I had 2 embryos transferred. Bhcg was 1980 on day 20 after ET; approx 2600 on day 22 and 4200 on dAy 24. Some of bleeding is red. Resumed Crinone 3 days after hpt, due to spotting/bleeding. I'm wondering if I can stop crinone...
ReplyDeleteI recommend continuing on the Crinone until you are at least 10 weeks gestational age. By that time, the placenta takes over hormone production and it should be fine to stop. Some fertility specialists will continue this until 12 weeks (end of the miscarriage risk period).

I underwent the mock transfer on Sept. 10, which happened to be day 16 or 17 of my cycle (I have 31-33 day cycles), and I am still spotting--it went from red to brown, and while it seems abundant on toilet paper, it's not enough to actually make it to my underwear. But still, we're talking 9 straight days of spotting. Does this indicate that anything is amiss? Does it suggest that anything should be tweaked when it comes time to do actual IVF? I'm planning on starting my first IVF cycle in about 5-ish weeks, depending which protocol they put me on (long or short).

It is probably some minimal endometrial trauma from the catheter. The transfer should be done more delicately and carefully.

ReplyDeleteIt did not rise sufficiently and would be a little worrisome that the pregnancy is not developing correctly. I would recommend that you continue to follow the bHCG levels on an every 48 hr sequence to see how it progresses. But you cannot make absolute determinations regarding the pregnancy just based on bHCG levels.

after that the spotting stoped.

with hard cramping... i take like a medicin duphaston folic acid
and cyclogest and aspirin ( i am 32 )
it can be sign of miscariage or i lose one embryo??

There is no way to know what is causing the spotting but in general, because it is so common, I tell my patients not to worry about it unless the bleeding is heavy like a period and bright red blood. It may be nothing, and the pregnancy will proceed normally, or it could be a indication that the pregnancy is abnormal. There is no way to know in advance. You will just have to wait and see how it goes.

DeleteBy now you are far enough for a blood pregnancy test so that needs to be done. It is possible that the bleeding is due to the Crinone, as I have seen this occur often. Also, the fact that it is not a large amount of bright red blood is reassurring. My recommendation is to ask your doctor for a blood pregnancy test today.

I'm over five weeks pregnant hcg levels have been more than doubling ever couple of days. Had blood and was nervous so doc did understand and we seen the Sac. That night I had more blood, red, and again Saturday but not enough too fill a pad. That wad this past Friday so today, Sunday I took two home pregnancy tests that came back with instant positive results. I don't know what to think and I'm so nervous.
ReplyDeleteThere is no way to know exactly what is going on. Some bleeding is indicative of an abnormal pregnancy and possible pending miscarriage, but some is not and the pregnancy can continue normally. At this point, you'll just have to wait and see how things go.

Hi doctor, i had a five day embryo transfer of 3 beautifl healthy embryos on the 22/04/13. My bhcg test was due for 6/05/13 so had it and d result was positive. On d same day, had a hpt and saw a faint line.waited and had another hpt and saw a faint line again. However today i started bleeding heavily so went for another bhcg test and d result is negative..i am beyond devastated..just wondering wat may have gone wrong?
ReplyDeleteIf the pregnancy test was a blood pregnancy test, then you had what is known as a "chemical" pregnancy which is an early miscarriage. I cannot tell you what happened, that is something that only God can know. In most cases of a chemical pregnancy it is due to an abnormal embryo so the pregnancy did not continue. It is the body's method for quality assurance. The good thing to keep in mind is that you showed that you can get pregnant by IVF. Now it is just a matter of getting a perfect embryo and you will be successful. As to other details about your cycle, I cannot give any advice without a review of your medical records. That can be done via Email but there is a fee for that service.

ReplyDeleteA small amount of bleeding does not mean that you are not or cannot be pregnant. Many pregnancies have a small amount of bleeding early on and continue to be successful.

Hello and congratulations. I advise my patients to disregard any discharge, spotting or bleeding that is less than a regular period and not accompanied by the same type of cramping. The signs you have may be normal. I cannot predict if the pregnancy will continue normally, but at this point I don't think it is worrisome and occurs quite commonly. With my patients, I attribute these symptoms to vaginal progesterone. Are you taking vaginal progesterone?

ReplyDelete14 days would be enough time for a home pregnancy test to turn positive, as well as, a blood pregnancy test. If both are negative, it indicates that you are not pregnant. So the bleeding and cramping is probably your period.

I have been spotting since 11/6 on and off but, I started to have light period bleeding on and off with tiny clots three days ago. should I worry? I will go in for antther blood test tomorrow.
I see early implantation and 1st trimester bleeding and often the pregnancy continues without problems. The only bleeding I worry about is if it is heavy like a menses, but spotting or light bleeding may not necessary be a bad sign. The only thing that can be done at this point is to take a wait and see approach. Following the bHCG's is one way to make sure the pregnancy is progressing normally.

ReplyDeleteI had my transfer on 2nd April. Beta HCG test done on 12th april showed 219. But on the day of beta onwards started light spotting with mucus. This turned to light bleeing with small clots and mucus .. Doctor suggested dupahastone two times . I am already on crinone gel,progynova and aspirin... Bleeding stopped for sometime .. Then if I engage in household activities it returns .. I have slight lower back pain .. Do I need to take bed rest ?
Hi. It is possible that the slight bleeding is a result of the Crinone. I have seen it many many times and don't worry about it. It has been found to be common with Crinone. It probably is because it causes a little friability of the cervix. I generally tell my patients not to worry about bleeding unless it is bright red and heavy like a period.
DeleteIn terms of bed rest, many studies have shown that this has no influence on the outcome of the pregnancy. However, if it will make you feel better emotionally and psychologically, then I would not dissuade you from doing it. Good Luck

ReplyDeleteBy 10 dpt you should have had a blood pregnancy test to see if you were pregnant or not. The bleeding could certainly be a miscarriage, your period or cervical bleeding from a continuing pregnancy. The blood pregnancy test is the first step.
Deletehi dr, my friend had 6 embryos implanted on 14/7, 26/7 she had some brown discharge and her bloods were negative, she then started bleeding on the 28/7 but it seems to be a constant bleed , her period is only usually 5 days she stopped bleeding today 6/8 . is this a failed ivf or is there still hope, she had been taking the progesterone suppository 3 times a day but finally gave up and stopped taking them on the 2/8 she is depressed and distraught, she had this done abroad and I was wondering if the flight back could have caused her bleeding, as she had no spotting b4 she flew back

DeleteIf her blood pregnancy test was negative, then she is not pregnant. However, the other possibility is pregnancy but she may have miscarried. There is no way to know which direction she is headed without doing a blood pregnancy test. The travel did not cause this.

DeleteHopefully, by now you already know the outcome of this question, but in answer for my readers, the only way to know if this bleeding is significant at this point is to do a blood pregnancy test or an ultrasound. Bleeding does not necessarily mean that a miscarriage has occurred.
I am 10 days out from a a 5dt and started having some brown and light pink spotting with minimal cramping. I went for hcg pregnancy test and it came back positive with a level of 42, two days later I went and it was 67. Is this pregnancy a failed pregnancy? Are these normal levels, please help Im so worried this pregnancy is going to end.

DeleteBy now you probably know the outcome of your pregnancy but for the sake of those that read my blog, the bHCG has to increase an minimum of 60%. Doubling is just a guideline. The only way to know if the pregnancy continues is to follow the bHCG levels and then an ultrasound when it is appropriate.

ReplyDeleteOur surrogate had 2 donor egg blasts transferred on April 28th with a positive beta of 215 on May 8th, and 547 on May 11. On May 10th, she had some slight spotting which then stopped. May 14th, yesterday, in the morning, she noticed dark brown spotting on her panty liner and light pink spotting on toilet paper. This progressed to some light red bleeding with a small clot noticed in the toilet. She describes having mild cramps but not like period cramps. Is it possible that one of the embryos could have implanted and is now being lost? Could there still be a progressing pregnancy? She is going for a repeat beta this morning and we are awaiting results. Thanks
I usually counsel my patients not to worry about bleeding unless it flows like a period and is accompanied by period-type cramping. Sometimes the bleeding can come from the cervix if she is using progesterone suppositories. At this point there is no way to predict what is going to happen, but in my experience, I've had many patients (including my wife) who have had heavy bleeding yet the pregnancy progressed normally and delivered a normal child. So, try not to worry to much at this point. Only time will give us the answers.

DeleteDr. Ramirez, I had an embryo transfer on July 4th. On July 7th I started spotting with a brown discharge and some small clots. On July 14th my pregnancy test was positive with a hcg of 36. On the 16th still pregnant with hcg of 126. On July 20th my hcg levels were 836. However, I'm still bleeding, every day since July 7th, and have tiny clots. Any ideas on what this can be? I do have a history of a friable cervix and stage 4 endometriosis. I've even had to have my organs separated as they adhered together.
ReplyDeleteHello. If you are using vaginal progesterone, that often will cause a little erosion of the cervix (not visible) and cause some spotting. In general, I advise my patients to not worry about spotting/bleeding unless it is red flow like a period and accompanied by cramping. This has nothing to do with the endometriosis.

However my progesterone was low and since then i take primolut depot injections twice a week. I also take 3 cyclogests suppositories everyday. I also take progyluton 3 times a day. Today for the first time i was disappointed to find brownish-darkish blood as i was in the bathroom. Should i be worried? My ultrasound isn't till 10 days from now. does medication cause bleeding?should i be expecting more?

ReplyDeleteMy presumption is that the testing you mentioned were blood pregnancy tests. If they were initially positive and then negative, then you are in the process of a miscarriage we call a chemical pregnancy.
DeleteBeta hcg 103 on 9/23/15, level increased to 266 on 9/25/15 and 10/7/15 was 21,323. Started spotting on 10/8/15, do I had an ultrasound on 10/9/15. Was told everything was great, 6 weeks 2 days, heartbeat 112. Still spotting light pink to brownish when I wipe. Should I be concerned? I'm using progesterone vaginal suppositories 200mg tid. I've read that they can cause cervical irritation and make cause bleeding.

Doc did a ultra scan on 40th day and confirmed 1 sac and could hear heart beat. She advised me to take HCG injection 3 days once and everytime i take HCG injection, i bleed after 72 hours

DeleteI would strongly urge you to see your doctor for more tests and evaluation. Having had an ectopic pregnancy in the past puts you at increased risk of another ectopic, so that is the main worry. Otherwise, the bleeding is not significant so it is a secondary problem at this point.
I had FET on 12/3. Had TW blood test on 12/14. Hcg was 54. Nurse said it was low and pregnancy will not progress. 2nd blood test was 44. Nurse said stop meds. Did not stop meds and took 5 HPTs since then and they all showed a Big Positive. Anybody been through this and achieved a successful pregnacy??? Need encouragement
ReplyDeleteAs long as the value is above 20, then an HPT will continue to show positive, but that may not indicate that this is a successful pregnancy. The blood pregnancy test should be followed to see how it progresses.
DeleteThank for the feedback. As of now the last HPT was negative so it was obvious that the pregnancy ended. I've had what appears to be a period. My dr has do the final blood work and I am waiting on the results. The Dr wants to start the 2nd cycle of the FET cycle in February. I will using frozen donor embryos. Any thoughts things that I can do different this time. By the way I am 46. I am afraid if another disappointment.
ReplyDeleteHi. You are asking a protocol question and that I cannot answer because every doctor does things differently. You'll have to trust that to your doctor as to what to do differently. However, you must also keep in mind that your age, which means the age related egg quality, is a major factor in regards to a successful pregnancy. Your risk of miscarriage due to an abnormal embryo is very high. I hope that you will be an exception to that rule. Good Luck
Deletei am 4 weeks 5 days pregnant with ivf. Had positive beta hcg results with levels doubling after 48 hours. Doctor called me for ultrasound a week after positive result . I started bleeding that day with bright thin red color each time i went for toilet.Doctor did ultrasound and yolk sac was seen but no baby was there. very next day bleeding stopped after a brown discharge. I mean bleeding lasted for only half a day. Doctor advised for bed rest and stopped cleaxane injection, ecosprin tablets and vaginal progestrone tablets. Suggested 100 ml Progestrone injections daily instead of vaginal tablets.Is everything fine or there is a matter of some worry. Please reply
ReplyDeleteHi. I don't think you can draw any conclusions yet. In my experience, a small amount of bleeding is fairly common in early pregnancy, possibly due to the progesterone suppository, and not worrisome. I tell my patients not to worry unless they have a heavy flow like a period with cramping. The ultrasound you had was very early and the main purpose is to rule out an ectopic pregnancy. That was accomplished. The fetus is generally not seen until 6 weeks gestational age at the earliest and viability is made with an 8 week ultrasound. I think you can still look forward optimistically.

ReplyDeleteI'm not sure how often you still reply, but thought I'd try asking here anyhow, as my clinic has no after hours care. I am currently 15dp5dt, and had a positive beta of 166 at 10dp5dt. My clinic doesn't do follow up betas with a result over 100 at 16dpo. I'm wondering if you could clarify how heavy cramping needs to be before it gets concerning. Since yesterday I've had some pink spotting off and on, and today I've had light cramping and ongoing redder, liquidy spotting. Nothing heavy, but I'm concerned because of the accompanying light cramping. I've had light cramping and occasional spotting since 3dp5dt, but this feels a bit more pronounced (but not even close to menstrual cramps). Does this sound more like miscarriage material? (I know there's no way to know for sure, but does the light cramping make things more ominous?)

Oh, I forgot to mention, I do have a very sensitive cervix which bleeds easily, and I'm on Endometrin 3x/day, so I'm not surprised I have some spotting, it's the accompanying light cramping which is concerning me most

ReplyDeleteMy wife underwent ET on 14th Apr. 2016. She had bleeding bit heavy for 5days after 12th day of ET. 1st BHCG was 18. 2nd HCG level after days dropped to 13. Is there any chance for pregnancy still?
Hello. I know this is late for a reply and you probably know the outcome by now. However, for those who follow this blog, a drop in bHCG is a bad sign and usually indicates that the pregnancy will fail.

My beta will be 2 days from now can anyone advice me please cause Im nervous it did not work again this time..

ReplyDeleteI had 5dbt on 8/9/16 of 2 5aa and 4aa embryos however 8 days later I started to bleed heavy off and on like a period for 3 days. My hcg level was drawn on 8/19 and the level was 114. then 2 days later it went up to 236. My question is could I have lost one embroyo and the other one is still there.? I would appreciate you expertise.

ReplyDeleteI did a one embryo transfer (day 2 four cell) on the 19th Sep. Bleed heavily (heavier than normal menses) on the 30th Sep onwards (day 11 after transfer) with very bad back ache . Did hcg test on same day with 7.8 reading. Did another hcg on the 3rd Oct with reading 37.8. Doc is asking to see me only on the 11th Oct to determine if it is ectopic. Have you encountered such cases before? Am still bleeding but its lighter like spotting and feeling dull pain in lower left abdomen. Is the chances of ectopic very high?

ReplyDeleteWe recently had frozen embryo transfer of 2 good quality embryos. We tested positive and are approx 6wks today. 2 days ago my wife started to bleed. It was around 10pm on Friday she passed 3 clots and the bleeding eased my morning. It's now very light and pale. Almost stopped. On Friday I rang our doctor who said to increase our progesterone pessaries from 1 in morning and night to 2 in morning and night. We are to go for blood tests tomorrow but sure so worried. Is this the usual signs of miscarriage or something less sinister?

Hello doctor, I have gone through ICSI I had 4 embryos transferred they were all day 2 ones. On 14th day after transfer I had brownish discharge & two days later light blackish bleeding it is sort of medium flow not the usual period type today is the 5th day it is even more less almost stopping but today passed a very small nail size clot what could that mean? I had not lost hope before but now I m abit scared plz help.
